APPLYING TO MARTIN LUTHER HOMES

Here at Martin Luther Homes we realise that looking for an aged care facility can be a daunting process, we realise when you are having to relocate yourself or find a new home that this is a major life decision for you and your family and we at Martin Luther Homes are here to help you with this process.

We have a dedicated caring team of staff that are here to assist you in this process. We can help you with the guidance you need when moving in to a home like ours and we are here to help with your delicate transition period.

Assessing your eligibility for aged care:

Before moving into an aged care facility in Victoria you must be assessed by an Australian Government Department through the ageing assessment team known as ACAT or ACAS.

ACATs are teams of health professionals, such as doctors, nurses or social workers, who provide information, advice and assistance to older people who are having a difficult time living in their own home.

Referrals for an ACAT assessment can be made by anyone. ACATs cover all of Australia and are based in hospitals or in the local community.

You may also wish to see your own doctor first who will also be able to refer you to your local ACAT.

The assessment is free of charge. A person with an ACAT approval for low care will need to be reassessed when their care needs change, particularly if they have not received care within 12 months of receiving the approval.

ACAT assessments are valid for up to 12 months. You will receive a copy of your assessment in person or by mail. If you are assessed as eligible, you should keep a copy of this assessment, as it is the approval for your move to the residential aged care facility.  If you require any assistance with your ACAT assessment don't hesitate to phone Martin Luther Homes for more information.

Working out the cost: 

Here at Martin Luther Homes  we understand that working out the financial requirements of entering into an  aged care facility like ours  can be confusing and we are happy to arrange a meeting with our friendly Chief Executive Officer Mrs Birgit Goetz to explain the fee structure.  

If you are interested in applying for residency at Martin Luther Homes, we will provide you with an information pack. 

Applicants will go on our waiting list according to their circumstances and the applicants will be contacted when an appropriate place within our facility becomes available.  

Moving in:

When a person moves into an aged care facility like ours we realise, that they must adapt to a new environment, where there are many new surroundings and people. Here at Martin Luther Homes we try to make that transition as quick and comfortable as possible and try our best to help our new residents feel very welcome.  Our experienced team spends time with new residents, their families and carers to ensure that the transition will be as smooth as possible.
